This privacy policy explains how I, Sara Bevins HCPC registered Chiropodist/Podiatrist, will use and protect any information that you give me when you use this service.

I am committed to ensuring that your privacy is protected.
Should I request from you certain information by which you can be identified when using my services, then you can be assured that it will only ever be used in accordance to this privacy statement.

This policy applies to the practice, and services offered by Sara Bevins within her Chiropody/Podiatry practice.

Clients that receive Chiropody/Podiatry treatment from Sara Bevins will be issued with a paper copy of the Privacy Policy at their initial consultation. This will contain the following information and they will be asked to sign a consent form stating that they understand and agree with how the information is collected, stored and held before treatment can commence.

Personal information I collect:

• Name and date of birth
• Address
• Preferred contact information (telephone and/or email)
• Occupation
• Medical history relevant to Podiatry treatments
• Medical conditions
• GP details and medication details.

Erasing your information:

All emails and texts will be removed once the appointment has been completed.
All written documentation will be securely stored for a minimum of five years after last consultation. The reasons being; for reference if you need to return for future treatments, as well as for my insurance and legal obligations.

How I store information:

• Personal hand written details are kept in my filing cupboard in my surgery, which is locked when I am not in attendance. I do not currently hold client’s details on computer at present but as and when this changes you will be informed and consent obtained.

• Email addresses: Email addresses are stored on my email account and are only used in response to the client’s initial enquiry for an appointment.

• Texts: These will have the client’s name only stored on my smart phone/contact numbers, no addresses or other personal information is stored and these are again used in response to client’s enquiry for an appointment.

Documents Held:

• Client/patient record card
• Client consent form

How I may share your information:

The only parties that potentially I could share your information with is likely to be another healthcare professional e.g. your GP/consultant but this would only be done in consultation with yourself regarding on-going referral for treatments beyond my professional remit.

Your rights:

• Right to access: You have a right to ask for a copy of your personal information, free of charge, in an electronic or paper format. You also have a right to ask me to amend or change any incorrect information about you.
• Right to be forgotten: You have the right to ask me to erase any information that I hold about you. This includes any personal information that is no longer relevant to original purposes. Or if you wish to withdraw your consent. In all cases and when considering such requests, these rights are obligatory unless it’s information that I have a legal obligation to retain.
• Data Portability: As the client you have the right to receive your personal information which you previously provided, also you have the right to transfer that information to another party.

For purposes of the General Data Protection Regulations (GDPR) 2018, the data controller is Sara Bevins HCPC registered Chiropodist/Podiatrist.
